ABSTRACT

@#AIM: To investigate the anxiety, depression and sleep status of patients with primary angle-closure glaucoma(PACG)and primary open angle glaucoma(POAG).<p>METHODS: Retrospective clinical study. The Anxiety Self-Rating Scale(SAS), Depression Self-Rating Scale(SDS)and Pittsburgh Sleep Quality Index(PSQI)were used to evaluate 60 patients with primary glaucoma(30 PACG, 30 POAG)diagnosed in the department of ophthalmology in our hospital during the period from June 2019 to December 2020 and 30 healthy adults from the physical examination department of our hospital as a control group. The scores of anxiety, depression and sleep quality in the three groups were compared.<p>RESULTS: The scores of SAS, SDS and PSQI in PACG and POAG groups(48.40±9.302, 53.40±8.625, 9.57±2.861; 42.57±9.684, 48.80±10.320, 7.23±2.223)were significantly higher than those in control group(37.03±6.805, 38.63±7.881, 4.87±2.688)(<i>P</i><0.05). The scores of SAS and PSQI in PACG group were higher than those in POAG group(all <i>P</i><0.05). With SAS≥45, SDS≥50 and PSQI>7, the positive rates of anxiety, depression and insomnia in the PACG group(77%, 73%, 70%)were higher than those in the POAG group(43%, 40%, 37%)and the control group(13%, 10%, 20%)(<i>P</i><0.05).<p>CONCLUSION: Patients with PACG has more severe anxiety, depression and insomnia than POAG, so necessary psychological intervention should be given.

ABSTRACT

OBJECTIVE@#To investigate the status of anxiety and depression in patients requiring emergency treatment during the epidemic of COVID-19 to identify the patients with acute psychological stress disorder.@*METHODS@#During the COVID-19 epidemic, the medical staff divided the patients visiting the emergency department into suspected group, fever group and control group through interview of the patients at triage. Self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were distributed to each patient, and a trained medical staff was responsible for assisting the patient to complete the scales.@*RESULTS@#A total of 557 sets of scales were distributed, including 211 in suspected COVID-19 case group, 167 in fever group and 179 in the control group. A total of 516 scales were retrieved, including 197 in suspected case group, 151 in fever group and 168 in control group. In the 3 groups, the incidence rates of anxiety and depression were 57.87% and 58.88%, 48.34% and 43.71%, and 18.31% and 18.99%, respectively, and the rates were significantly higher in suspected group and fever group than in the control group ( < 0.01), and significantly higher in suspected group than in fever group ( < 0.05). The standardized anxiety and depression scale scores in suspected case group, fever group and control group were 57.38±16.25 and 42.58±14.27, 51.23±15.29 and 38.32±15.39, and 32.58±17.8 and 12.25±12.94, respectively. Compared with the control group, both suspected case group and fever group had significantly higher standard scores for anxiety and depression ( < 0.01), and suspected case group had significantly higher standardized scores than fever group ( < 0.01).@*CONCLUSIONS@#Among the patients visiting the emergency treatment, the patients with suspected COVID-19 and common fever are more likely to develop anxiety and depressive symptoms.

ABSTRACT

OBJECTIVE@#To observe the effect of ginger-separated moxibustion on fatigue, sleep quality and depression in the patients with chronic fatigue syndrome.@*METHODS@#A total of 62 patients with chronic fatigue syndrome were randomized into an observation group (31 cases, 3 cases dropped off) and a control group (31 cases, 2 cases dropped off). In the control group, the patients had normal diet and proper physical exercise. In the observation group, on the basis of the control group, the ginger-separated moxibustion was added at Zhongwan (CV 12), Shenque (CV 8) and Guanyuan (CV 4), 30 min each time, once every two days, 3 times weekly. Separately, before treatment and after 4 weeks of treatment, the MOS item short form health survey (SF-36), the Pittsburgh sleep quality index (PSQI) scale and the self-rating depression scale (SDS) were adopted to evaluate the degrees of fatigue, sleep quality and depression in the patients of the two groups.@*RESULTS@#In the observation group, the score of each item of SF-36, the score of each item of PSQI and SDS score after treatment were all improved significantly as compared with those before treatment respectively (<0.05, <0.01). In the control group, the scores of overall health, vitality and mental health in SF-36 and the score of sleep time of PSQI after treatment were improved as compared with those before treatment respectively (<0.05). After treatment, the score of each item of SF-36, the scores of sleep quality, sleep time, sleep efficiency and sleep disorders of PSQI, as well as SDS score in the observation group were all better than those in the control group respectively (<0.01, <0.05). The score of SF-36 was relevant to the scores of PSQI and SDS in the patients of chronic fatigue syndrome ( =0.331, <0.05; =-0.706, <0.01). The improvement value of SF-36 score was closely related to the improvement value of SDS score in the observation group ( =-0.657, <0.01).@*CONCLUSION@#The ginger-separated moxibustion effectively relieves fatigue and depression condition and improves sleep quality in the patients with chronic fatigue syndrome. The fatigue condition is relevant with sleep quality and depression condition to a certain extent in the patients.

ABSTRACT

Objective To investigate the correlation between daytime sleepiness and anxiety,depression in patients with obstructive sleep apnea and hypopnea syndrome(OSAHS).Methods A total of 117 patients with sleep -snoring from September 2015 to September 2017 who admitted to the Department of Respiratory Medicine of Shengjing Hospital Affiliated to China Medical University were collected for general information.ESS,SDS,SAS questionnaire and polysomnography were detected.The differences between simple snoring and OSAHS group were compared,and the correlation between Epworth Sleepiness Scale (ESS),Self-rating Depression Scale (SDS),Self-rating Anxiety Scale(SAS) score was analyzed.Results The scores of ESS,SDS and SAS in the OSAHS group were (16.24 ± 3.82) points,(46.27 ± 9.01) points,(48.21 ± 9.44) points,respectively,which were higher than those in the simple snoring group [(6.0 ± 2.58) points,(35.50 ± 18.40) points,(36.55 ± 17.97) points] (t =-14.425,-2.521,-2.780,all P < 0.05).There was a correlation between SDS or SAS score and ESS score in OSAHS group(r=0.419,0.313,all P <0.05).Conclusion The sleepiness and anxiety,depression in OSAHS patients is more severe than simple snoring;ESS score in OSAHS patients can reflect the severity of the disease,the degree of sleepiness in patients with OSAHS is related to depression and anxiety.

ABSTRACT

Objective: To investigate the pschological status of the patients with oral lichen planus disease (OLP) . Methods: 100 patients with OLP and 100 healthy controls were included. The self-rating anxiety scale (SAS), self-rating depression scale (SDS), neuroticism extraversion openness (NEO) and WHOQOL-BREF were used to analyze the life quality and pschology of the subjects.Andrenocor-cicotropin (ACTH) and cortisol (CORT) in serum were measured. The relationship between OLP and mental illness, stress, personality traits and life quality was analyzed. Results: There were no significant differences of gender and age between the case group and the control group (P> 0. 05) . There were significant differences of the SAS score, SDS score, NEO score, WHOQOLBREF score and the level of ACTH and CORT (P < 0. 05) in serum between the 2 groups. Logistic regression analysis showed that anxiety, depression, personality and life quality were the influencing factors of OLP prevalence. Conclusion: The development of OLP may be related to psychological status, depression, stress, anxiety and so on.

ABSTRACT

Introduction: We started the gender-specific clinic for women to provide sufficient treatment for female patients. The purpose of this study was to clarify the characteristics of patients using the gender-specific clinic for women, and to assess the association among depression, physical and mental subjective symptoms.Methods: This observational study included female patients aged 16-84 years who visited our clinic between June 2012 and December 2015 (N=97). In addition to general attributes, we collected data on physical and mental symptoms, and depression status using the Cornell Medical Index (CMI) and Self-rating Depression Scale (SDS), respectively, at the first visit. We conducted analyses to assess patient characteristics and the association between subjective symptoms and depression, and between physical and mental symptoms by estimating odds ratios (ORs) and 95% confidence intervals (CIs).Results: The average age of subjects was 50.4 years. The average CMI score was 42.7 points and 55.9% of the subjects were suggested to be neurotic. The average SDS score was 45.0 points and 64.0% of them were suggested to be depressed. The association with depression by SDS was observed in subjective symptoms of CMI such as fatigue (OR [95%CI]: 7.66 [2.26-25.99], p-value: 0.001) and anxiety (OR [95%CI]: 11.73 [1.80-∞], p-value: 0.006). Physical symptoms in the cardiovascular system were positively association with some mental symptoms such as tension.Conclusion: As female patients often have mental symptoms, it is essential for doctors engaging in gender-specific medicine for women to approach patients while considering psychological and mental aspects.

ABSTRACT

<p><b>OBJECTIVE</b>to observe the effect differences and safety between acupuncture and sham acupuncture for female depression on the basis of conventional western medicine therapy.</p><p><b>METHODS</b>Forty-two patients with depression who met the criteria were randomly assigned into an observation group and a control group, 21 patients in each group. The basic treatment in the two groups was fluoxetine. The patients in the observation group were treated with acupuncture at Zhongwan (CV 12), Xiawan (CV 10), Qihai (CV 6), Guanyuan (CV 4) and bilateral Shangqu (KI 17). Sham acupuncture was used at the above points in the control group. The treatment was given once a day on the first 3 days and once every 3 days after that, totally for 8 weeks. The scores of self-rating depression scale (SDS) and Montgomery Asberg depression rating scale (MADRS) were compared before and after 4-week and 8-week treatment in the two groups. The effects and adverse reactions were evaluated.</p><p><b>RESULTS</b>After 4-week and 8-week treatment, the SDS and MADRS scores were significantly lower than those before treatment in the two groups (all <0.05), and the scores in the observation group were lower than those in the control group (both <0.05). The improvements of SDS and MADRS scores in the observation group before and after treatment were better than those in the control group (both <0.05). The total effective rates of SDS, MADRS scores were 85.7% (18/21), 90.5% (19/21) in the observation group, which were better than 38.1% (8/21), 57.1% (12/21) in the control group (both <0.05). The adverse reactions in the observation group were significantly lower than those in the control group (<0.05).</p><p><b>CONCLUSION</b>Acupuncture combined with fluoxetine are effective and safe for female depression.</p>

ABSTRACT

Objective To investigate the influence of depression on glycemic control in the patients with type 2 diabetes mellitus(T2DM).Methods The Zung self-rating depression scale(SDS) was used to assess depression.A total of 276 cases of T2DM were divided into the group A(SDS standard score ≥53 points) and B(SDS standard score <53points).The levels of HbA1c,FPG,HOMA-IR,etc.were compared between the two groups,and the influencing factors of glycemic control in T2DM patients were analyzed.Results In the patients with T2DM,the SDS standard score was correlated with HbA1c(r=0.26,P<0.05).The multivariate regression analysis showed that the SDS standard score was still correlated with HbA1c (β =0.30,t =5.1,P< 0.05).The HbA1c level in the group A was higher than that in the group B(t=3.685,P<0.05);after correcting the factors of sex,age and education,the HbA1c level in the group A was still higher than that in the group B(F=47.8,P<0.05).Conclusion The depression mood is adverse to glycemic control in T2DM patients.

ABSTRACT

Objective To investigate the supportive care needs of liver cancer patients after interventional therapy and its influencing factors. Methods A total of 228 patients with primary liver cancer treated by interventional therapy were included. General demographic characteristics and clinical data were collected, and a questionnaire survey was conducted using 34 items of cancer patients' supportive care needs, a brief questionnaire(SCNS-SF34), Self-rating an Anxiety S cale(SAS)and a Self-rating Depression Scale(SDS). Multiple regression models were used to analyze the impact factors of demand domain score. Results The score of psychological needs, health systems and information needs, physiological and daily life needs, patient care and support needs, sexual needs of patients with liver cancer intervention were 3.15±1.31, 3.08±1.16, 2.96± 1.44, 2.60 ±1.09, and 2.60 ±1.09, rescpectively. Anxiety and depression were found in 37.19% and 46.31% of patients, respectively. The average scores of SAS and SDS were 46.79 and 51.49. Multiple linear regression analysis showed that the psychological needs of patients with scores of age and education were negatively correlated. SAS score of patients with mental health score system and demand, information demand and demand areas were positively correlated. Women patients with high SDS scores, has high score of physical and daily life needs . The score of patient care and support were positively correlated to clinical requirements in the field of TNM stage. Conclusion The medical staff should support the intervention for the different demands and the bad mental state of the patients after liver cancer interventional surgery, and effectively improve the patient's compliance and improve their quality of life.

Objective To investigate the depression of patients with Meniere disease after surgery ,and the relationships to vertigo and the improvement of the quality of life .Methods A total of 46 patients with Meniere dis-ease and 95 patients with other diseases of otorhinolaryngology (as control) were enrolled in this retrospective stud-y .They underwent operations between 2013 and 2015 .They finished self -rating depression scales ,Meniere dis-ease outcomes questionaire by snail mail .They were asked about the frequencies of vertigo before and after surgery . The gender ,age and postoperative course in the control group were matched to those of in the Meniere group .The relationships among vertigo ,the improvement of the quality of life and postoperative depression were compared .Re-sults In the Meniere group ,69 .77% of patients did not have depression ,while 13 .95% were with mild depres-sion ,16 .28% with moderate ,none with severe .Depression was more severe in the Meniere group than in the con-trol group .There were no obvious linear relationships between vertigo and depression or between quality of life and depression .There was no statistical difference in depression of patients with different vertigo frequencies .Conclusion There is still a certain degree of postoperative depression in near 30% of patients with Meniere disease after sur-gery .This may not be related to vertigo nor the quality of life .

Objective To study the living quality and mental health status in losing-single-child family couples .Methods A total of 841 losing-single-child family couples and 674 family couples with children alive conforming to the entering group condition were random sampled from the three districts of Chongqing main city .The on-site survey was performed by using the general condi-tion questionnaire ,90 Symptom Checklist (SCL-90) ,Self-rating Anxiety Scale (SAS ) ,Self- rating Depression Scale (SDS ) ,Social Support Rating Scale (SSRS) ,Memorial University of Newfoundland Scale of Happiness (MUNSH) and WHO Quality of Life Scale (WHOQOL-BREF) Chinese version .Results There were no differences in gender ,age and culture between the losing-single-child family couples group and family couples with children alive group (P>0 .05);the SCL-90 total score and each factor score , SAS and SDS score in the losing-single-child family couples group were significantly higher than those in family couples with chil-dren alive group(P<0 .05) ,the SSRS score ,MUNSH score and WHOQOL-BREF Chinese version evaluation score were signifi-cantly lower than those in the family couples with children alive group(P<0 .05);the multivariate regression analysis showed that the four regression equations in the social relation field ,environment field ,depression factors and total happiness had statistical sig-nificance (P<0 .05) .Conclusion The living quality ,mental health status and subjective well-being in the losing-single-child family couples are significantly affected ,timely psychological crisis intervention and persistent psychological support and mental health ed-ucation should be give them to improve their psychological health status ,increase the living quality and subjective well-being .

ABSTRACT

Objective To observe the clinical efficacy of moxibustion in treating perimenopausal syndrome (PMS).Method Totally 108 PMS patients of yang deficiency or yin deficiency constitution were randomized into a treatment group of 56 cases and a control group of 52 cases. The treatment group was intervened by moxibustion, while the control group was by medication. The modified Kupperman Index (KI), Hamilton Anxiety Scale (HAMA), Symptom Checklist-90 (SCL-90), and Self-rating Depression Scale (SDS) were observed before and after treatment for comparison.Result The KI score, HAMA score, SCL-90 total score, and SDS score were significantly changed in both groups after intervention (P<0.01,P<0.05). After treatment, the KI score, HAMA score, SCL-90 total score, and SDS score in the treatment group were significantly different from that in the control group (P<0.01,P<0.05).Conclusion Moxibustion is effective in treating PMS, and it can improve the anxiety and depression symptoms of the patients.

ABSTRACT

OBJECTIVES:This study aimed at identifying psychiatric problems among patients who were diagnosed with breast cancer and scheduled for mastectomy.METHODOLOGY:All the women who were diagnosed with breast cancer and were awaiting their scheduled mastectomy at the surgical clinic were interviewed using the Self-Reporting Questionnaire (SRQ) and the Zung Self Rating Depression Scale. Their sociodemographic and histopathological reports were also noted.RESULTS:There were 21 women included in this study with mean age of 43.5 years. Majority (61.9%) were married and mostly high school graduates (42.8%) and unemployed i.e. 76.19% housewives. Most complained of breast pain (71.4%) while a third (38.1%) noted nipple discharge. Based on the SRQ,61.9% scored positive for mood/ anxiety and only 4.7% were found to be positive for psychosis. When rated according to the Zung Self Rating Depression Scale, 6 or 28.6% were noted to have marked depression and 5 or 23.8% had severe depression.CONCLUSION:Since most women with breast cancer in this study have been found to be positive for mood/anxiety and depression, it is recommended that screening for psychiatry morbidity should be part of the standard care and a support group manned by mental health professionals should be established as part of the holistic management.

OBJECTIVES: There is a controversy regarding the construct validity of anxiety and depression. Some believe that these two symptoms are basically the same construct, that is, both measure what is called 'general distress' or two phenomena are distinct experiences which often coexist. To further understand relationship between anxiety and depressive symptoms, we investigated the factor structure of a combined anxiety and depression scale among psychiatric outpatients. METHODS: Data of Zung's Self-Rating Depression and Anxiety Scales were gathered from 401 newly visiting psychiatric outpatients at a university-affiliated hospital. We performed a component analysis on the 40 items from two scales. RESULTS: Exploratory factor analysis revealed a seven factor structure explaining 56% of total variance. Overall finding indicated that depression and anxiety scales consisted of four symptom domains : mainly depressive symptoms, mainly anxiety symptoms, common somatic symptoms, and others. CONCLUSIONS: Our results suggest that the construct of self-reported depressive and anxiety symptoms are more complex than previously thought, i.e., either one or two factor theories. These findings also support that anxiety and depression can be better modeled by dimensional approach. Clinicians may be alert for the fact that both depression and anxiety scales measure distinct and also common aspects. Further researches on other scales especially, interview based instruments are needed.

ABSTRACT

Objective To observe the effect of electroacupuncture on onset time and effectiveness of Paroxetine for mild or moderate depression patients and on the scores of Hamilton depression rating scale for depression(HAMD) and self-rating depression scale(SDS). Methods Fifty-five patients with mild or moderate depression were randomly divided into A group( Paroxetine group,29 patients) and B group (Electroacupuncture combining with Paroxetine group,26 patients). Therapeutic periods for the two groups were 6 weeks. HAMD and SDS were used to evaluate the efficacy before the research and after treatment on 1 week,2 weeks,4 weeks,6 weeks.Results HAMD scores and SDS scores for A group were significant difference after 2 weeks treatment compared with the beginning of the research (P< 0.05 ) ;HAMD scores and SDS scores for B group were significant difference after 1 week treatment compared with the beginning of the research(P<0. 05 ). Total effective rate of HAMD for A group was 71.4% ,total effective rate of SDS for A group was 75% ,total effective rate of HAMD for B group was 88% ,total effective rate of SDS for B group was 84%. The total effective rate between the two groups ordered by the individual chi-square test was significant difference( X2 = 4. 1790, P= 0. 0408). Conclusion Electroacupuncture can shorten the onset time and increase the effectiveness of paroxetine for mild or moderate depression patients.

ABSTRACT

Objective To investigate the sleep features in the patients with irritable bowel syndrome (IBS) and compare the sleep quality between those IBS patients who were with and without anxiety and depression.Methods Pittsburgh sleep quality index questionnaire (PSQI), self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were measured in the 145 IBS patients and 59 regular physical examination volunteers.IBS patients were also divided into two subgroups-patients with or without anxiety and depression based on cutoff scores of SAS and SDS.Comparisons of sleep quality were made between subgroups, and between IBS patients and volunteer controls.Results Compared with the controls,the SAS raw score, SDS raw score and SAS positive incidence in IBS patients were shown statistically significant differences (P < 0.05 ), while the SDS positive incidence had no statistically significant difference(P > 0.05 ).PSQI total scores were significantly higher in the IBS patients without anxiety and depression (P < 0.05), 3 domains (sleep quality, sleep disturbances and daytime function disorder) were also found statistically significant differences ( P < 0.05 ), compared with the controls.The IBS patients with anxiety and depression were statistically significantly different from the controls ( P < 0.05 ) in 6 domains (sleep quality, sleep latency, sleep efficiency, sleep disturbances, sleep time and daytime function disorder) and significantly higher PSQI total scores( P < 0.05 ).Statistically significant differences (P <0.05) were also found in all 7 domains and with higher PSQI total scores in IBS patients with anxiety and depression, compared with IBS patients without anxiety and depression.Conclusions IBS patients were more likely to have sleep abnormality, mainly in sleep quality, sleep disturbances and daytime function disorder and PSQI total scores.The abnormalities of these factors were independent of emotional disorder.However, emotional disorder worsened the sleep disorder in IBS patients.

BACKGROUND: It is a well known fact that students majoring in medicine and engineering suffer from higher stress level and depression than students in other majors. This study was done to assess the prevalence of depression and the infl uencing factors among the university students majoring in either medicine or engineering and to compare the depression prevalence between the two groups. METHODS: The study randomly selected 640 students either majoring in medicine or engineering from two universities in Kyung-Gi province, Korea. Each of 320 students majoring in either of the two majors from each university was recruited. Among 640 students, 360 students, who were composed of 172 medical students and 188 engineering students was finally chosen for the analysis. The Zung Self-Rating Depression Scale was used to measure the depression state. RESULTS: The average Zung depression score of university students was 49.2 alpha 9.8. Medical students had a higher score of 52.4 alpha 9.3 as compared to engineering students, who scored 46.3 +/- 9.4. Overall depression rate was 42.8%. Medical students had a higher depression rate (52.3%) than engineering students (34.0%). The logistic analysis showed that infl uencing factors on depression state were sex, grade, major, stress and satisfactory sleep. The main source of stress for medical students was the grades from classes whereas job search was for engineering students. There were differences between the two groups in managing the stress. Medical students chose alcohol drinking or smoking for stress management whereas engineering students playing selected video games as their outlet. CONCLUSION: Medical students from urban areas seemed to be depressed more than engineering students. Their stress management methods among the subjects were drinking, smoking, and playing video games which may cause further physical and psychological problems in the future.

ABSTRACT

As mental stress has a great impact on the symptoms of atopic dermatitis, we administered Qi-related prescriptions to 6 patients with atopic dermatitis. Not only were mental and autonomic imbalances, but also skin conditions were improved in all six. We examined changes in palmoplantar sweating and a self-rating depression scale to evaluate the effectiveness of the Qi-related prescriptions. The Qi-related prescriptions were used in combination with standard treatments as follows : keishikaryukotsuboreito in Case 1, shigyakusan in Cases 2 and 3, yokukansankachimpihange and keishikaryukotsuboreito in Case 4, shigyakusan and keishikaryukotsuboreito in Case 5, and saikokeisikankyoto in Case 6. It appears to be important to treat Qi-disorders, along with skin disorders, in patients with atopic dermatitis.

ABSTRACT

The purpose of this study was to evaluate the effect of keishibukuryogan (KB) against the cognitive symptoms associated with silent brain infarction in a prospective cohort study. The subjects were 93 patients with silent brain infarcts who visited the Department of Japanese Oriental Medicine, University of Toyama, and its allied hospitals. They consisted of 24 males and 69 females, mean age (± S.E.) 70.0±0.8.Group SK (n=51) consisted of patients who used KB extract for more than6months per year. Group SC (n=42) consisted of patients who did not use Kampo formulas. The NS group (n=44) consisted of elderly subjects who had no silent brain infarction, 21 males and 23 females, with a mean age (± S.E.) of 70.7±0.7 years. Among the three groups, the revised version of Hasegawa's dementia scale, apathy scale and self-rating depression scale were compared between the study start and after three years. In the SK and SC groups, these scores, and the subjective symptom levels (head heaviness, headache, dizziness or vertigo, stiff shoulder) were also studied. The results showed that the self-rating depression scales at study start for the SK and SC groups were significantly higher compared to the NS group. In spite of the scores for the NS group increasing after three years, the SK group scores were significantly decreased compared to the SC and NS groups. KB was effective against head heaviness, which often complicates silent brain infarction. In the above mentioned, KB was effective in treating cognitive disorders and subjective symptoms related to silent brain infarction.
